Fill out the Water Sample for Bacteriologic Examination form, collect a sample per the posted instructions, and bring it to the lab (samples must be submitted before 3:00 pm Monday-Thursday for coliform, Monday-Wednesday for HPC) with the $12 fee paid prior to testing.
