Submit the online PFAS Testing for Private Wells Request Form and NHDES will determine whether your well qualifies for free agency sampling based on proximity to known PFAS contamination.
Private well owners may submit a request, but NHDES compares the well location against its own contamination records to determine sampling eligibility and priority; submitting a request does not guarantee the well will be tested.
